Students requesting a change or correction of name must present a photo ID as well as legal documentation, such as a court order, marriage license, birth certificate, or a passport and this form to the Office of the Registrar. IMPORTANT : If you change your name after the first day of finals during your graduation quarter, then you will be required to pay a diploma fee if you wish to have your new name on your diploma.
